Achievement C.V. Proforma. A form that captures employee’s main achievements in their role.

Certificate of Completion. Specimen certificate to present to delegates at the end of a course.

Course Attendance Form. Records attendance details of delegates.

Course Booking Procedure. An example of a booking procedure for delegates who are geographically remote from the training centre.

Course Enrolment Form. A proforma for delegates (and their line managers) to enrol for a training course. Also can be used as joining instructions and setting post course follow up work.

Learning Resource Centre Leaflet. A folded A4 leaflet to market the company’s Learning Resource Centre to employees.

Learning Resource Feedback Sheet. A proforma to capture feedback from users of a company’s Learning Resource Centre.

Personal Development Plan proforma. An A4 form capturing details of employee’s pledges towards self development.

Post Course Development proforma. A method of capturing action plans from courses.

Probationary Review proforma. A specimen 3-month probationary review sign off form for new starters.

Project Plan. A specimen project plan for a team leadership course.

Team Leader/Manager Development Plan proforma. An A4 sized document that gives a ‘road map’ for training for a team leader.

Training & Development Policy. A sample statement of policy.

Training Equipment on Loan proforma. A4 landscape signing-out/in form for loaning equipment to other departments.

Training Planner. Two A4 sized proformas to assess requirement for training and record attendance at training over a period of time.

Writing Pre and Post Course Work. A handout for line managers to help plan out pre course objectives and write follow up action plans.
